We provide requested medical records to patients and outside providers upon request. We also process medical records addendum requests. In some cases, there is a fee assessed as follows:
Requests for records to be pushed to MyChart or sent to the provider directly are free.
The Medical Records office is located inside the hospital and is available by appointment or walk-in. To schedule an appointment, please call 530-841-4121.
To request access to medical records, please fill out the request form and return it to Fairchild Medical Center.
