Admission and Registration
Your physician will arrange your admission and notify you of the date and time to come to the hospital. When you arrive at the hospital, check in with our volunteers and wait to be called. Bring your social security number, insurance card, Medicare or medical group insurance information with address and Advance Directives. If you do not have Advance Directives, you will be provided with information and necessary forms for you to complete should you wish to establish Advance Directives.
Our registration clerk will complete the required forms. You will be required to sign the admission form. Your signature authorizes the medical staff and hospital personnel to provide you with care. If you are under the age of 18, a parent or legal guardian must sign for you. Should you require surgery or need a special diagnostic procedure, you will be asked to sign a separate consent form or forms.
HELP PAYING YOUR BILL
Patients requesting financial assistance may contact financial services at (530)-841-8537 and ask to speak with a financial counselor. The integrity of clinical decision making is not affected by compensation arrangements. This means the decision to receive treatment or to remain in the hospital is based on medical necessity, not on payment arrangements.
ADVANCE DIRECTIVES
California and federal law give every competent adult, 18 years or older, the right to make their own healthcare decisions, including the right to decide what medical care or treatment to accept, reject or discontinue.

SAFE SURRENDER SITE

Valuables
We urge you to keep all money, valuables and items of sentimental value at home or to leave them with your family. If you wish temporary storage of valuables, your nurse can place items in the hospital safe. Fairchild Medical Center cannot accept responsibility for valuables left in your room.
Discharge Process
Discharge planning is a part of your care. As a patient at Fairchild Medical Center, you arrange to leave the hospital when your physician determines that it is medically appropriate for you to go home or to another care setting.
Your physician, nurse and other members of your healthcare team will work with you and your family to plan your discharge.
Advice Nurse
Fairchild Medical Center provides free health information to our community through a program called Advice Nurse. Please call 1-877-529-5604 to speak with a nurse. If you are having a medical emergency, call 911 or go to the nearest emergency room.

Through Advice Nurse you may receive information about an illness or healthcare concern. We hope you find this service helpful.
For Visitors
At Fairchild Medical Center, we understand that health problems can be stressful for patients as well as their family and friends, so we strive to offer comfort and convenience during this difficult time. Visiting hours vary within different areas of the hospital and are also at the discretion of the medical or nursing staff.
Visiting hours for the majority of the hospital are 8 a.m. to 8 p.m. All visitors should use the Main Entrance.
Intensive Care Unit and Obstetrics have special visiting hours based on the care of the patient needed at that time. For visiting hours of those departments, please contact that unit.

At Fairchild Medical Center, it is our goal to always provide professional, caring and quality care. Our Patient Safety/Risk Management Department is here if you or a family member has a concern related to care, treatment, services or a patient safety issue.
