Your financial support of the Fairchild Medical Center Foundation helps us to support Fairchild Medical Center and as a result makes a lasting impact upon the health of our community. Programs such as our Employee Scholarship Program, Mammogram Scholarship Program and Healthy Kids Fair have far reaching positive effects on our community.
We invite our donors to select how their gift is used. You may choose to give an unrestricted gift that will be used where the need is greatest or designate your gift to a restricted purpose such as our building fund, scholarship funds or special projects such as our Healthy Kids Fair or Stop the Bleed.
The Fairchild Medical Center Foundation is a 501(c)3 tax exempt organization.

The Fairchild Medical Center hosts a series of annual events to raise money, to educate and to support our local community. Please click on our event logos for more information about each of our special events.
In our pursuit to support the efforts of Fairchild Medical Center to provide continued access to quality health care in our rural area, the Fairchild Medical Center Foundation believes in supporting FMC employees wanting to further their education and abilities through continuing education and certification.
Each year the Fairchild Medical Center Foundation awards multiple scholarships to employees from various hospital departments. Full-time and part-time FMC employees who have been with FMC for six (6) continuous months are eligible to apply. All applications are reviewed and must be approved by the employee's direct manager, hospital administration and the FMC Foundation Board of Directors.
The purpose of the Scholarship Fund of the Fairchild Medical Center Foundation is to provide assistance to hospital employees interested in advancing their education.
Awards will be limited to $1,500 per employee, per calendar year. If you are participating in a multi-year program you will need to apply again each year for additional funds.
Scholarship recipients will have a period of one calendar year in which to receive and utilize their awarded monies. Monies not distributed within a calendar year will be considered forfeited and the application/award withdrawn.

Applications on average take 30 to 60 days from submission to approval/denial.
* Scholarship Funds are available to full and part time employees only who have been continuously employed by FMC for a minimum of 12 months.
The generous financial support we receive makes it possible for the Fairchild Medical Center Foundation to fulfill our ongoing mission of supporting Fairchild Medical Center. Our Pinnacles of Giving members are a critical part of the impact we are able to make in our rural community.
Every year we host an annual invitation only event for our members who donated $500 or more in the previous calendar year as well as for our Lifetime Donors who have given $5,000 or more to the Foundation.

The Fairchild Medical Center Mammography Fund exists to provide limited payments for mammography screening for residents of Siskiyou County. These monies are to be used by women or men who do not qualify for other financial aide programs due to age and/or medical history, and patients who are uninsured, underinsured or are without sufficient funds to cover the services.

This fund is sponsored and supported by Fairchild Medical Center and Radiologists Dr. Michael Maloney and Dr. Peter Halt.
FUNDING GROUPS
Funds for the Fairchild Medical Center Mammography Fund are obtained through fundraising activities in the community and through donations from our community members, businesses and organizations.
CRITERIA
A formal referral for services must be initiated by the patient’s physician.
Patient must have household income at or below 200% of the Federal poverty level.
Patient must be uninsured or underinsured –
Patient has only Medicare part A
Or Patient has private insurance with barriers such as restricted benefits, large deductibles, or co-payments
Mammogram screening must be done at Fairchild Medical Center.
Patients must reside in Siskiyou County.
PROCEDURE
Referral for services must be initiated by the patient’s physician to Fairchild Medical Center. Upon completion of the Mammography Fund application form, Fairchild Medical Center will review patient income and establish eligibility for the Mammography Program. Fairchild Medical Center assumes no responsibility in validating accuracy or reported income by patient. Subsequent payment processing is handled by Elizabeth Langford, Executive Director for Fairchild Medical Center Foundation.
As a special way to commemorate the birth of your child, the Fairchild Medical Center Foundation offers our families the opportunity to purchase a commemorative package as part of the Footprints to the Future program. Your child's footprint will be placed on our Footprints to the Future wall inside the hospital and you may select from a variety of footprint packages to purchase.
For more than 20 years, the Fairchild Medical Center Foundation has offered our Legacy Brick program as a way to honor members of our community and our team as well as family and friends. Legacy Bricks are placed along the pathways leading to the main entrance of the hospital.
