PROVIDER VERIFICATION

Dear Requesting Facility:

We provide online “Hospital Affiliation Verifications” through MDQuery. This online database will allow you to query and print verification letters of our current and previous providers.

Please contact the Medical Staff Office at (530) 841-6207 or (530) 841-2049 to obtain a login and password. When you receive your login and password you may use the following information to access our website:
MDQuery

Once you are logged in, enter at least one of the ID items requested:

• Last name (or first few letters of last name)
• First name (or Last four digits of social security number)

If you cannot locate a provider, please contact the Medical Staff Office.
