PROVIDER VERIFICATION

Dear Requesting Facility:

We provide online “Hospital Affiliation Verifications” through MDQuery. This online database will allow you to query and print verification letters of our current and previous providers.

Password: Please email the Medical Staff Office: Hope Stansbury at hstansbury@fairchildmed.org or Kristi Apodaca at kapodaca@fairchildmed.org to obtain MDQuery password. When you receive your login and password you may use the following information to access our website:

Once you are logged in, enter at least one of the ID items requested:

• Last name (or first few letters of last name)
• First name (or Last four digits of social security number)

If you cannot locate a provider, please contact the Medical Staff Office.
